Application Form With PDF Confirmation - Stran 1

Below is an example of an application form. If you would like the respondent to receive confirmation of his application on his e-mail address, you can set it on the right, under the option 'Alerts from responses quick settings' or in the tab 'EDIT' - 'Settings' - 'Notifications' - 'Completed survey'.

In the 'Thank you' section you can also set for respondents to receive a confirmation in PDF format.

The application form or URL of the application form can be sent to desired e-mail addresses. You can also embed it on your website. First, you must choose the 'Embed' skin in the 'EDIT' - 'Design' tab, and then in the 'PUBLISH' - 'Url' tab choose 'Insert code into your own webpage (Embed)'.

You need to activate the application form before publishing.

If you need more information in your application form, simply add questions from the left menu.
